Admission Requirements
For the 2026-2027 school year, the Department of Education (DepEd) has streamlined the admission process under DepEd Order No. 017, s. 2025. The goal is to make enrollment more inclusive and “paperless” where possible.
Required Documents
The “once-in-a-lifetime” submission rule is now in effect. If you submitted these documents in previous years, they should already be in the Learner Information System (LIS).

Early Registration for S.Y. 2026-2027
At Sorsogon National High School, students and parents no longer need to visit the campus in person because the early registration for incoming Grade 7 and Grade 11 learners is now online.Â

JHS Special Programs Application Forms
Simply click the link that matches your classification below to download the application form for the specific program you are interested in. Once downloaded, please fill out the form completely and bring the hard copy to the Sorsogon National High School Annex Building, along with the other necessary requirements to finalize your registration.
Reminder: When visiting the Annex Building, please ensure you have all supporting documents ready, such as your PSA Birth Certificate or Report Card, and adhere to the school’s Code of Conduct regarding appropriate attire and grooming.
SNHS Request Forms
If you require any official school documents such as your SF10 (Form 137), SF9 (Form 138/Report Card), Certificate of Good Moral Character, Diploma, or other certifications. Please download the request form provided below. Fill out the hard copy completely and bring it to the Registrar’s Office located at the Main Building for processing.
